CHANGE IN STATUS POLICY

Prior approval is required for any significant change in the research plan (as described in the original application), including:

• Termination of PBF Fellowship
• Transfer to different institution
• Change in Mentor
• Significant change in proposed studies

A transfer or change in Mentor requires the following actions:

1. Write a letter to the PBF Fellowship Program Director and...
   
   • Explain the reason for change and the date of transfer
   • Provide name of new Mentor
   • State current Mentor will continue as a secondary Mentor
   • Detail how PBF Fellowship research studies will continue
   • Have current Mentor co-sign letter
     
2. Ask new Mentor to write a letter indicating the guidance and support provided for new studies
3. Include a bio-sketch of new Mentor
4. Complete Change in Status form >>
5. Provide a letter from new department chair if change in institution is requested

A significant change in proposed studies requires the following action:

1. Write a letter outlining reasons for change in research plan and new research aims
2. Both Fellow and Mentor must co-sign the letter
