Applications, including letters of reference, must be received no later than Monday, October 12, 2009, at Midnight PST.

APPLICATION INSTRUCTIONS

  • Complete and mail the Face Page, including original signatures. It must be received by the PBF Program office in Seattle by October 12, 2009. Access Face Page (a PDF document) >>
  • Access the online Application >> and complete the Candidate Information questions.
  • Complete the online application form. Work can be stopped and saved, and changes made at any time until “submit” is clicked at the end.
  • From within the online application, click on the word “Application” to download the Word document.
  • Convert the Word Application and save it, along with all the appendix materials, as a single PDF file.
  • Upload completed Application (as a single PDF file) as directed at the online application site.

Three letters of recommendation, in sealed envelopes with original signatures, must be received by the Fellowship Program office in Seattle by October 12, 2009. They should include:

  • demonstrated potential for establishing an independent pulmonary research career
  • evidence of originality
  • adequacy of scientific background and evidence of superior performance
  • quality of research publications

NOTES

  • Print the help Checklist and PBF Fellowship Application forms to use as references.
  • Carefully follow all instructions. An application will not be considered if it: (1) is received after the deadline; (2) is incomplete; (3) does not comply with format or page limit guidelines; or, (4) is not submitted as one PDF file.
  • Use Arial 11-point font size, margins of 0.5 inch and single-line spacing for all text.

SUPPLEMENTAL SUPPORT

Applicants may e-mail a brief (two-page limit) notice of updated information (i.e., published papers, grant awards, other relevant news, etc.) to dsnapp@u.washington.edu at the PBF Fellowship Program office by January 4th. Send as a single PDF file.
